Office Policy

All new patients are kindly urged to arrive 30minutes prior to your appointment. For established, returning patients, please arrive 15minutes prior to your scheduled appointments. We pride ourselves on respecting your appointment times as scheduled.  A courtesy phone call of your intention to cancel an appointment is requested 24hours in advance.

Please provide photo proof of identity, your insurance information, and an updated medication list upon arrival. Billing questions are best answered by our billing staff.  In the event of any billing concerns not satisfactorily answered by our billing office, please do not hesitate to ask our staff directly.

Please notify our staff if you were recently admitted to a hospital or if you have had blood work drawn with another physician prior to your appointment.

Upon admission to the hospital, call our office immediately to notify our staff of your admission and request your admitting doctor order a nephrology consultation with your kidney specialist.  Our mission and commitment to you is to provide the best, most comprehensive continuity of care in the outpatient or inpatient setting.
